# Inland Empire Independent-Provider Census

**Scope:** Independent (non-Kaiser / non-Loma Linda University Health / non-Optum / non-Dignity / non-Prime / non-Tenet / non-HCA / non-Providence) physician groups, IPAs, multispecialty groups, FQHCs, and independent specialty practices across **San Bernardino and Riverside counties** (the Inland Empire), assembled to map potential alignment / clinically-integrated-network (CIN) partners and aggregation targets for Redlands Community Hospital (RCH).

**Headline count:** **188 independent provider organizations** retained after deduplication and exclusion of dominant-system entities.

## Methodology & caveats

- **Sources merged.** Seven regional research files (`_build/independents/{west_sb, central_sb, east_sb, pass, nw_riverside, sw_riverside, coachella}.json`) were merged with the previously hand-classified 38-group eastern-corridor file (`independent_groups.csv`). The three input schemas (some used `city`, others `base_city`; some used `provider_count` vs. `provider_count_est`; panel-lives lived under `est_panel_lives` or `panel_lives_est`) were normalized to a single schema.
- **Underlying data.** Group identification rests primarily on the CMS **NPPES** NPI registry (organization NPI-2 records), the California **DMHC** capitated-provider / risk-bearing-organization (RBO) list, **IEHP** IPA directories, CMS Medicare revalidation/enrollment data, and practice websites/corporate filings. Most rows cite NPPES and/or DMHC.
- **Dedupe rule.** One record per **name + city**. A group operating multiple sites *within one city* is a single record with a sites note. The same parent group operating in **different cities** is intentionally kept as separate name+city records (e.g., Cajon Medical Group appears in San Bernardino, Redlands, Moreno Valley, and Temecula), each annotated with a `MULTI-SITE` cross-reference so the parent relationship is visible. One true cross-file duplicate (Inland Eye Institute, Colton) was collapsed.
- **Independence screen.** Only groups genuinely independent of Kaiser, LLU Health, Optum/UnitedHealth (incl. NAMM, PrimeCare, Beaver, IFMG), Dignity, Prime, Tenet, HCA, and Providence are retained. **Seven excluded entities** were dropped on the merge: Optum Care Network–San Bernardino (formerly PrimeCare), Inland Faculty Medical Group (Optum/NAMM), the Arrowhead Regional/CUSM county faculty group, and the four Optum-owned SW-Riverside entities (Perris Valley MG, Murrieta Valley Family MG, PrimeCare of Hemet Valley, Optum–Murrieta Primary Care).
- **Borderline / flagged independents (≈40 records).** Some retained groups carry ownership nuance and are flagged in `affiliation`/`notes` for verify-before-aligning, e.g.: **LaSalle Medical Associates IPA** (Astrana/APC ~25% minority stake; still physician-governed), **Chaparral Medical Group** (Akido Labs MSO tie), **Riverside Medical Clinic** (UHS MSO management affiliation), **Rancho Family MG** and **TriValley MG** (PE-backed — LightBay / Webster), **Altais Medical Group–Riverside** (Altais/Brown & Toland enablement model), **Desert Oasis Healthcare** (subsidiary of Heritage Provider Network), **Eisenhower Health Medical Associates** (employed group of an independent nonprofit hospital — market leader, not a target), and several CVMG/ProHealth/Inland Healthcare entities that surface on Optum's *contracted-network* directory but are not on Optum's *owned* roster. None of these are owned by an excluded system, so all are retained, but each needs a direct ownership/contracting confirmation before outreach.
- **Counts are estimates.** Provider counts, PCP counts, and panel-lives are **estimates** (mostly ranges) inferred from NPPES NPI density, site counts, and group self-description — not audited rosters. For network IPAs (LaSalle, DOHC, DAP, Clinicas) the figures are **network-wide**, not RCH-corridor-attributable. Treat all numbers as directional.
- **Geocodes.** Lat/lng are rooftop where available; several are city-center approximations (flagged in notes).

## Region-by-region

### West San Bernardino — Rancho Cucamonga / Fontana / Ontario / Chino / Upland (14)
A multispecialty-and-primary-care belt (7 multispecialty, 7 primary-care; no FQHCs surfaced here). The strategically relevant names: **Chaparral Medical Group** (large legacy multispecialty, 40–80 providers, but flagged for an Akido Labs MSO tie), **Cucamonga Valley Medical Group (CVMG)** (6-site primary-care group; appears on Optum's contracted directory — verify), and the **San Antonio Regional Hospital (SARH)–aligned** entities (CARE4U / Sierra San Antonio) — independent of the excluded systems but tied to a same-tier independent community hospital, so partner-or-competitor, not roll-up. Solid small-group targets: **Chino Hills Family Medical Group** (clearly physician/PA-owned), ProHealth Partners, Our Family Medical Group, Chino Medical Group. This region sits farthest from Redlands and is the most Optum-contested; lower priority for an RCH CIN.

### Central San Bernardino — San Bernardino / Colton / Highland / Loma Linda / Grand Terrace (42)
The single densest pocket of independents (tied with the Redlands corridor). Mix skews to **specialty (14)** and **primary-care (11)**, plus **8 FQHCs**, **4 multispecialty**, and **3 IPAs**. The marquee target here is **Physician Health Network (PHN) IPA** — a ~30-year, physician-owned, risk-bearing multispecialty IPA (600+ providers, ~54,000 lives) and one of the cleanest large independent aggregation counterparties in the county. Also significant: **LaSalle Medical Associates IPA** (Dr. Albert Arteaga's 300K–360K-life Medi-Cal network; minority Astrana stake — flagged) and **All United Medical Group IPA** (Colton). Safety-net depth is real: 8 independent FQHCs (H Street, Mt. Vernon, Oasis, IBHS, Central Neighborhood, Blessing, RSBCIHI Grand Terrace, plus Unicare/Neighborhood from the CSV). The 399 E Highland and 1800 Western MOB clusters concentrate independent cardiology, OB/GYN, GI, and endocrinology practices that map directly to RCH service lines.

### East San Bernardino / Redlands Corridor — Redlands / Yucaipa / Calimesa (42)
RCH's home market and the most strategically important region — heavy on **primary-care (17)** and **specialty (18)**, anchored by the named flagship groups: **Community Alliance Medical Group (CAMG)** — founded 2024 by Arrowhead Orthopaedics' Dr. John Steinmann *explicitly to rebuild local independent primary care*, the single most aligned CIN target; **Redlands-Yucaipa Medical Group (RYMG)** — a 23-year physician-owned multispecialty group that **left Optum/Epic Management and returned to independence** (~2024–25), now an ally rather than a gatekeeper; and **Maren Medical Group** — Dr. Arteaga's brand-new (June 2025) independent PC group, a pathway back toward LaSalle's IPA infrastructure. The specialty bench around the RCH campus (Terracina/Industrial Cir/Olive Ave) is exceptional: **Arrowhead Orthopaedics** (27+ surgeons, the largest independent ortho group in the IE), independent cardiology, GI/endoscopy, OB/GYN (incl. **Icon Medical Group**, on-campus), oncology, colorectal surgery, urology, neurosurgery (Cortez), and dermatology. Plus several risk-bearing IPAs HQ'd in Redlands (**MedEx Health Network**, **SoCalHealth Inc.**) and the multi-site **Cajon Medical Group** PC.

### San Gorgonio Pass — Banning / Beaumont / Cherry Valley (17)
A thin-coverage corridor where independent primary care is scarce and valuable. The flagship find is **Rancho Paseo Medical Group** (Banning, founded 1992) — the dominant independent multispecialty + ancillary group native to the Pass (SDA *name/council* affiliation only, not LLU ownership); the top Pass aggregation target. Otherwise small independent FM/IM practices (AHA, Inspire, Devadas Moses, R. Douglas Collins), an OB/eye specialty or two, and three safety-net FQHCs (**Neighborhood Healthcare–Beaumont**, **RSBCIHI–Banning** tribal consortium, **IBHS–Banning**). One caution: **Mariam Moghadam MD** sits on the 81 Highland Springs Optum/Beaver + LLU campus — verify she isn't Beaver/Optum-contracted before counting her.

### Northwest Riverside — Riverside / Corona / Norco / Jurupa Valley / Moreno Valley (15)
Headlined by **Riverside Medical Clinic (RMC)** — 180+ providers across 7 offices, physician-owned, but flagged for a 2024–25 UHS MSO management affiliation (still not KP/LLU/Optum). Other multispecialty: **Citrus Valley/Norco Medical Group** (contracts through IFMG/Optum for capitation but isn't system-owned — flag), **Aspen Medical Group** (independent since 1988), and **Altais Medical Group–Riverside** (Altais/Brown & Toland enablement model — flag). Smaller independents (Raincross, Volta, Advanced Health, ABC, Alertive, 365 MDCare) plus one FQHC (Central City–Norco). This region is geographically far from Redlands and largely oriented toward Riverside-Corona hospitals — lower corridor relevance.

### Southwest Riverside — Temecula / Murrieta / Hemet / Menifee / Lake Elsinore / Perris (34)
The second-largest regional bucket and the most IPA-rich outside central SB. The marquee names: **Rancho Family Medical Group** — the largest independent group in SW Riverside (150–250 providers, 15+ sites; PE-backed by LightBay; an exclusive *referral-only* affiliation with UC San Diego Health, not an acquisition) and **PromiseCare Partners** — the 2022 unification of four long-standing independent IPAs (HCMG/MVCMG/TVPMG/FSMG), self-described as the largest IPA network in the IE and explicitly distinct from Optum's PrimeCare of Hemet Valley. Add **TriValley Medical Group** (PE-backed, physician-led), **Seven Star Health** (unusually broad independent specialty bench in Hemet), **American Primary Care Alliance IPA**, **Prime Partners IPA** (confirmed *not* on Astrana's roster — note: unrelated to Prime Healthcare the system), Meadowview IPA, and Trinity multispecialty/GI. A long tail of small Hemet/Temecula/Menifee/Lake Elsinore PC practices and independent specialty groups (cardiology, GI, heme/onc, OB/GYN, urology) rounds it out.

### Coachella Valley — Palm Springs / Rancho Mirage / Palm Desert / Indio / Coachella (24)
Far from Redlands and dominated by two large independent (non-excluded) platforms rather than roll-up targets: **Desert Oasis Healthcare (DOHC)** — the valley's largest non-Optum managed-care/IPA platform (150K–200K lives), but a **Heritage Provider Network** subsidiary and itself a regional consolidator; and **Eisenhower Health Medical Associates** — the employed group of independent nonprofit **Eisenhower Health**, the valley's flagship hospital (market leader/benchmark, not a target). Genuinely independent risk-bearing IPAs do exist (**Empire Physicians Medical Group**, **Live Well IPA**). The east valley (Indio/Coachella) holds the cleanest physician-owned multispecialty targets — the **Curry-led Coachella Valley Care / Santa Rosa del Valle** pair — plus strong independent FQHC capacity (**DAP Health**, which absorbed Borrego; **Clinicas de Salud del Pueblo**; Central City; St. John's; CVVIM free clinic). Listed for completeness; the valley is a separate market, not an RCH corridor opportunity.

7. The Redlands-campus specialty cluster (independent cardiology, GI/endoscopy, OB/GYN incl. on-campus **Icon Medical Group**, oncology, urology, neurosurgery) — service-line co-management.

## Smallest / lowest-priority (tiny, niche, or out-of-market)
A large share of the 188 are 1–6-provider solo/small PC or single-specialty practices (e.g., concierge **In Your Best Interest/MDVIP**, **Coachella Valley Direct Primary Care**, solo DO/IM corporations across Banning, Beaumont, Hemet, Lake Elsinore, Cathedral City). These add little aggregated panel and are low-priority except for targeted **access** gaps (front-door PC in thin-coverage cities: the Pass, Calimesa, Desert Hot Springs, Cabazon/Morongo, east-valley farmworker communities). Independent urgent-care sites (Stately, Apex, Rapid Care, Singh MD, All Valley) and **FQHCs** are access/community-benefit partners rather than managed-care aggregation targets — valuable for coverage, not for risk-bearing lives. The **Coachella Valley and NW/SW Riverside far ends** are separate hospital markets; relevant for completeness and competitive context, but not RCH-corridor roll-up opportunities.
